Physical Therapist Assistant

JOB DESCRIPTION

Position Title:   PHYSICAL THERAPIST ASSISTANT
Department: Home Health/Hospice – Clinical Operations
Reports to: Therapy Manager

Position Summary:

Provides Physical Therapy designed to improve individual and/or family quality of life to a specified client group and their families. Coordinates comprehensive health care and community-based services, home visits, and assessments. Develops, implements, coordinates, and evaluates health care plans in an outpatient environment; collaborates with other health care providers and community service agencies to coordinate care.
 
Essential Job Functions:
Directs and aids the patients in active and passive exercises, muscle reeducation, and gait and functional training, activities of daily living including transfer activities and prosthetic training following the plan of care developed by the supervising Physical Therapist.
Essential Job Requirements:

EDUCATION:   Current PTA License
EXPERIENCE: Minimum of 1 year home health care or related experience
REQUIRED SKILLS: Detail oriented, organized, ability to multi-task
PREFERRED SKILLS: Self-starter, ability to work alone under little supervision
PHYSICAL REQUIREMENTS: Lift 50 pounds routinely
