Physical Therapist Assistant
JOB DESCRIPTION

Position Summary:
Provides
Physical Therapy designed to improve individual and/or family quality
of life to a specified client group and their families. Coordinates
comprehensive health care and community-based services, home visits,
and assessments. Develops, implements, coordinates, and evaluates
health care plans in an outpatient environment; collaborates with other
health care providers and community service agencies to coordinate care.
Essential Job Functions:
- Treat patients to relieve pain, develop or restore function, and maintain maximum performance, using physical means, such as exercise, massage, heat, water, light and electricity. For Example:
Directs and aids the
patients in active and passive exercises, muscle reeducation, and gait
and functional training, activities of daily living including transfer
activities and prosthetic training following the plan of care developed
by the supervising Physical Therapist.
- Observe, record, and report to the PT the patient’s reaction to treatment and any change in the patient’s condition. Prepares PT Intervention Note upon each treatment.
- Guide the patient in his/her use of therapeutic, creative, and self-care activities for the purpose of improving function.
- Instruct patients in care and use of wheelchairs, braces, crutches, canes, and prosthetic and orthotic devices.
- Instructs other health personnel and family members in certain phases of the therapy program of which they may assist the patient.
- Communicate with the Physical Therapist, Nurse Supervisor and/or primary nurse assigned to the patient, the need for a continuation of therapy services for each patient.
- Calls client/patient and schedules appointment for clinical visit. Must arrive in a timely fashion to client/patients home. If unable to do so, must call client/patient in advance, arrange different appointment times, and document on daily paperwork.
- Document each visit made to the patient and incorporates note into the clinical record.
- Prepares written discharge summary, with a copy retained in the patient’s clinical record and a copy forwarded to the attending physician.
- Participate in case conferences as required.
- May supervise home health aides if physical therapy is the only skilled service provided in lieu of a registered nurse.
- Ensures that the supervising PT performs supervisory visits
at least once every 5th visit.

| EXPERIENCE: | Minimum of 1 year home health care or related experience | |
| REQUIRED SKILLS: | Detail oriented, organized, ability to multi-task | |
| PREFERRED SKILLS: | Self-starter, ability to work alone under little supervision | |
| PHYSICAL REQUIREMENTS: | Lift 50 pounds routinely |
